May 11, 2021Hello, I am a physician assistant who is starting to see patients with PANS/PANDAS. I wanted to know if I could use these forms in my practice. Are there any royalties required to use these forms? I don’t want to infringe on anybody’s copyright but I think these forms would be a big help for me getting started. May 17, 2021Just a suggestion, audio record your appointments if legal in your state. Over 1/2 it is legal if 1 party is aware (you of course).
My sister used to do this when she was being treated with cancer and she found it very valuable. It’s a lot of information.
I have also made a binder with my sons full records, timeline, any evidence such as drawings before and after onset, and even a letter from his teacher. Diagnostic criteria and treatment protocol is in there also.
